*NEW Contract Attachment 6 Training Record -
This form is used by Providers to track their annual Early Intervention Trainings (12 Hours per Fiscal Year).
*NEW Contract Attachment 7 Provider Referral Checklist -
This form is to be submitted with the Providers Monthly Invoice for each child documenting the Inital Date of Contact & Date Services Began for each new referral / authorization.
*NEW Contract Attachment 8 Monthly Report -
This form is used by Providers to report child and family progress to the team. It is sent to Early Steps with the Monthly Invoice.

This form is used to document consultations between team members and a copy is submitted to Early Steps with the Monthly Invoice.

This form is used to document the individuals involved in a child's Team Review and should be completed by each Provider involved.

This form is used by Providers to submit the Monthly Billing to Early Steps for their Services.

This form is for our Providers and is used to document their travel for Early Steps appointments.

This form is used by Providers and Service Coordinators to document services completed.
This form is used by Providers to document progress and updates for their individual sessions.
Insurance Verification Form ESPAÑOL -
This form is used by Service Coordinators and/or Providers to collect insurance information for third party billing. The form is an Adobe pdf and can be edited on the computer, however the form must have original-ink signatures.

Local Early Steps Staff
Medical Fragile Enhancement Team 
A project created to increase the body of knowledge on medical conditions and therapies, as they relate to the development of medically fragile infants and toddlers.
Infant Mental Health Workgroup 
A workgroup intended to design multidisciplinary, interagency
strategies to develop networking and referral systems to
provide family oriented infant mental health services.
NEW Communication from a Child's Point of View
This Class is taught by Early Steps' Jeremi Grosser, MS, CCC-SLP 
& will be for families with children that have communication concerns. During the class the families will learn everyday strategies to increase & improve their child's communication skills & sessions will start back up in January.

Interested in becoming an Early Steps Provider?
Complete the following steps to breeze through the process.
1. If you do not have an National Provider Identifier (NPI) Number go
bottom of the web page.
4. Enter your First & Last Name and then create a personal Password (at least 6
characters) using at least one of each of the following: Upper Case Letter,
Lower Case Letter, Number, and Special Character
**Document your Password**
5. When your information is accepted you will move onto the First Page of the
CMS Application and a window will pop-up noting your Username.

6. Enter all the required information (*) in each section and
Select Save & Continue to move on.
7. Once your application has been completed, submitted, and
confirmed you will receive an email from the State Office
with your Provider Checklist attached.
